The discharge planning process is started at the time of admission. It is a dynamic, on-going process.

The treatment team will address the problems and concerns of the patient and will offer suggestions for the continuation of care after discharge. This may include but would not be limited to a partial-hospitalization program (PHP), an intensive outpatient program (IOP), outpatient visits with a private therapist, follow-up with a psychiatrist for medication management or AA/NA attendance.

Discharge occurs when the patient’s goals have been achieved, nursing and therapeutic interventions are complete and the physician deems the patient to be stable and ready to leave the hospital.
